Before You Visit
General Procedure
Prior to confirming the first appointment, Dr. Finch will reach out for a brief phone conversation in order to address any concerns you may have.
Initial visits are always consultations. Such visits last at least 90 minutes, and may be done in one single visit or two 45 minute visits. At times, a third 45 minute appointment may be required. Following a thorough evaluation, a treatment plan will be developed and recommendations provided.
Prior to your first appointment, please complete this packet of forms, which can be signed securely online. Alternatively, you may click here to download and print them in advance of your first appointment.
Important Policies
In general, for new appointments, payment is due in advance in order to confirm the appointment. For subsequent visits, payment is due at the time of visit, unless other arrangements have been made. Cancellations made at least two full business days in advance do not incur any charge; for cancellations made with shorter notice, the full visit fee will be charged unless it is possible to reschedule.
Please be advised that Dr. Finch does not participate in any insurance plans, but will provide invoices and receipts that can be submitted for out of network reimbursement if offered by your insurance plan. This out of network reimbursement guide may be helpful in navigating this process.
